First, register for a team of 4 golfers at this time.

Use the company name of the team captain. If your company has several teams playing, please give each team a unique name, like,"ACME Sales" and "ACME Yard" for example.

Please fill out each field in this order form.

After your purchase, the Chapter will contact your team captain to get your marketing materials.

Please type the size for each of your 4 players in the appropriately labeled field on the order form.

Use the naming convention as it appears in the chart below, eg, S, M, L, etc.
